Case Manager

Responsibility

The common responsibilities for this position include assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress, and facilitating interdisciplinary approaches. It involves completing intake interviews and reviewing therapist evaluations to determine clients’ requirements. The role requires establishing treatment programs by setting schedules, coordinating services, and arranging resources. Monitoring cases includes verifying clients’ attendance, observing and evaluating treatments, advocating for needed services, and intervening in crises. Maintaining clients’ records through reviewing case notes and logging events is essential. Additionally, the position entails communicating clients’ progress in weekly interdisciplinary meetings and evaluations, evaluating the application of learning to treatment results, and preparing reports by collecting, analyzing, and summarizing treatment data and trends.
